Common use
Ciloxan Ophthalmic Solution is indicated for the treatment of infections caused by susceptible strains of the designated microorganisms in the conditions listed below:
- Bacterial Conjunctivitis: Caused by Staphylococcus aureus, Staphylococcus epidermidis, Streptococcus pneumoniae, Enterobacter cloacae, Haemophilus influenzae, Proteus mirabilis, and Pseudomonas aeruginosa.
- Corneal Ulcers: Effective against Pseudomonas aeruginosa, Serratia marcescens, Staphylococcus aureus, Staphylococcus epidermidis, and Streptococcus pneumoniae. It is commonly prescribed by ophthalmologists for patients presenting with purulent discharge, conjunctival injection, corneal infiltrates, or epithelial defects where bacterial etiology is suspected or confirmed.
Dosage and direction
The dosage and frequency of administration depend on the severity of the infection.
- For Bacterial Conjunctivitis: Instill 1 to 2 drops into the affected eye(s) every 2 hours while awake for the first 2 days, then 1 to 2 drops every 4 hours while awake for the next 5 days.
- For Corneal Ulcers: Instill 2 drops into the affected eye every 15 minutes for the first 6 hours, then 2 drops every 30 minutes for the remainder of the first day. On day 2, instill 2 drops hourly. From day 3 through day 14, instill 2 drops every 4 hours. Patients should be instructed to avoid contaminating the dropper tip and to not touch the eye or any surface with it. If more than one topical ophthalmic drug is used, administer them at least 5 minutes apart.
Precautions
- This product is for topical ophthalmic use only; not for injection or oral use.
- Patients should not wear contact lenses during the course of treatment.
- As with other antibacterial preparations, prolonged use may result in overgrowth of nonsusceptible organisms, including fungi. If superinfection occurs, discontinue use and institute alternative therapy.
- Patients should be advised not to use leftover medication for subsequent infections without consulting a healthcare provider.
- Use with caution in patients with a history of hypersensitivity to quinolones.
Contraindications
Ciloxan Ophthalmic Solution is contraindicated in patients with a history of hypersensitivity to ciprofloxacin, to other quinolones, or to any of the components in this medication.
Possible side effect
Although generally well-tolerated, the following adverse reactions have been reported:
- Local ocular burning, discomfort, or pain upon instillation
- Conjunctival hyperemia
- Bad taste following instillation (due to nasolacrimal duct drainage)
- Crystalline precipitates (white crystals) in the superficial cornea, which usually resolve upon discontinuation of therapy
- Allergic reactions, including itching, swelling, or dermatitis of the eyelid
- Corneal staining, photophobia, tearing, or dryness
- Rarely, nausea and headache
Drug interaction
Specific formal drug interaction studies have not been conducted with Ciloxan Ophthalmic Solution. However, the systemic administration of some quinolones has been shown to elevate plasma concentrations of theophylline, interfere with the metabolism of caffeine, and enhance the effects of the oral anticoagulant warantin. Although the systemic concentration of ciprofloxacin following topical ocular administration is low, caution is advised when co-administering with these drugs.
Missed dose
If a dose is missed, it should be administered as soon as possible. However, if it is almost time for the next dose, skip the missed dose and resume the usual dosing schedule. Do not double the dose to make up for a missed one.
Overdose
Topical overdose of Ciloxan is unlikely to cause serious symptoms due to minimal systemic absorption. If accidentally ingested, symptomatic treatment is recommended. Flushing the eye with lukewarm water may be performed in case of excessive instillation. There is no specific antidote.
Storage
- Store at controlled room temperature 15°–30°C (59°–86°F).
- Protect from light.
- Do not freeze.
- Keep the container tightly closed when not in use.
- Discard any unused solution 4 weeks after opening the multi-dose bottle. Single-use containers should be discarded immediately after use.
